Paravertebral and Local Ozone Therapy: Application in Low Back, Neck, Muscle, and Trigger Point Pain

Anatomic source of pain

Low back pain can originate from the disc, facet joint, paravertebral muscle, nerve root, sacroiliac joint, or myofascial structures. Before application, the anatomic source of the pain should be determined.

How is it administered?

The pain map, radiation, neurological examination, muscle spasm, and trigger points are evaluated. When necessary, clinical findings are correlated with MRI or ultrasound. Sterile local application is performed on appropriate targets.

Investigated mechanisms focus on local inflammatory signals, redox balance, microcirculation, and the muscle spasm–pain cycle.

Relationship with lumbar herniated disc

In 2025 meta-analyses, there are data indicating short-term pain and functional benefits for intradiscal ozone; however, intradiscal application is not the same method as paravertebral injection.

Radicular pain

Clinical studies comparing paravertebral ozone with epidural steroid interventions report positive signals in the short and medium term. Larger comparative and long-term studies will clarify which patient groups may benefit more from which approach.

Neck and trigger points

Cervical disc, facet, trapezius spasm, and radicular pain should be differentiated. If there is loss of strength, progressive numbness, or signs of myelopathy, advanced neurological evaluation is the priority.
