Lumbar Disc Herniation and Ozone Therapy: The Difference Between Paravertebral and Intradiscal Ozone

Short answerThe strongest clinical data for ozone in lumbar disc herniation comes from imaging-guided intradiscal applications. There are also controlled human data for paravertebral ozone; these two applications are not the same in terms of technique, indication, and risk.

MRI and symptom correlation

Visualizing a disc protrusion/herniation on MRI does not mean it is the definitive source of pain. Disc, nerve root, facet, sacroiliac joint, muscle, and hip origins must be differentiated.

Paravertebral ozone

It targets the paraspinal muscles/soft tissues around the spine. In a small double-blind placebo-controlled study, positive results in pain/function were reported when added to standard treatment.

Intradiscal ozone

It is an interventional procedure performed directly into the disc under imaging guidance; sterile intervention, proper disc selection, and radiological targeting are required.

2025 meta-analysis

In 8 RCTs and 1,744 patients, intradiscal ozone was found to have favorable short-term results in VAS pain and ODI functional outcomes.

Surgical indications

If there is progressive muscle weakness, severe motor deficit, cauda equina signs, or severe functional loss despite appropriate conservative treatment, surgical evaluation should not be delayed.

New combination data

In a 2026 randomized study, the addition of periradicular ozone to endoscopic discectomy was associated with additional improvement in some parameters in the early period; this does not mean it is an alternative to surgery.
