IPL / BBL for Rosacea, Facial Redness, and Telangiectasia

Treatment of rosacea, persistent facial redness, and telangiectasia with IPL/BBL; a guide to vascular targeting, level of evidence, patient selection, and safety.

Moderate evidence

Short answerIPL/BBL is one of the most studied light-based modalities in reducing persistent erythema and superficial telangiectasias associated with rosacea. The goal is not to completely eliminate rosacea, but particularly to control the vascular and redness component.

In which findings is it more meaningful?

  • Persistent facial redness
  • Telangiectasias around the cheeks and nose
  • Persistent erythema following flushing episodes

Scientific Evidence

A 2024 systematic review evaluated 14 studies and reported that IPL yields positive results in rosacea, particularly for erythema and telangiectasia. Patient satisfaction is high in most studies; however, due to parameter heterogeneity and study quality, it is difficult to define a single standard protocol.

Acne-like papules and pustules?

Inflammatory lesions of rosacea are not treated with vascular therapy alone. If necessary, planning is conducted in combination with topical or systemic rosacea treatments.

Safety

Tanned skin, dark phototypes, a predisposition to melasma, and photosensitivity risk affect parameter selection. Appropriate eye protection is mandatory around the eyes.

Our Approach at Our Bodrum Clinic

We differentiate whether the redness is due to rosacea, sun damage, superficial vessels, inflammation, or other dermatologic causes. In appropriate cases, we use a vascular filter and a stepped energy approach.
