Laser Therapy in Biceps Tendon Pain | Biceps Long Head Tendinopathy in the Anterior Shoulder

The number of direct, strong studies on laser for biceps long head tendinopathy is limited; shoulder tendinopathy and general HILT meta-analyses support the adjunctive use of 980 nm in addition to rehabilitation.

Short answerDirect disease-specific data for high-intensity laser in biceps long head tendinopathy causing pain in the front of the shoulder is limited. A 2026 tendinopathy meta-analysis shows that HILT can reduce pain and disability, particularly in shoulder tendinopathies. This result is not direct evidence for the biceps tendon; laser should be evaluated as an adjunctive option in addition to load management and tendon exercise.

What is biceps long head tendinopathy?

The long head of the biceps tendon, which runs in the bicipital groove at the front of the shoulder, can become painful due to overload, rotator cuff issues, or repetitive overhead activities. Anterior shoulder pain may increase with lifting and pulling movements.

How can laser help?

Positive results on pain and function have been reported in HILT tendinopathy studies. However, there is no specific dose and protocol standard for the long head of the biceps.

The role of 980 nm

For superficial to mid-depth tendon targets, 980 nm is a more suitable primary wavelength.

The role of 1470 nm

It has a more superficial thermal character; it is not the primary option for targeting the deep proximal biceps tendon.

What does the scientific evidence say?

Level of evidence: B-. Direct HILT data exists for shoulder tendinopathies; because evidence specific to the long head of the biceps is limited, the results are indirect.

Our approach in our Bodrum clinic

In our Bodrum clinic, the biceps tendon, rotator cuff, subacromial structure, and cervical sources are evaluated together in anterior shoulder pain. If laser is used, it is combined with tendon loading and scapular-rotator cuff rehabilitation.

Frequently asked questions

Does biceps tendinitis resolve with laser?

Pain may decrease in some patients; however, tendon load capacity needs to be redeveloped.

980 nm or 1470 nm?

In this anatomy, 980 nm is the more rational primary option.
