Laser for Achilles Paratenon Pain | Superficial Tenderness Around the Achilles

Direct clinical data for laser in Achilles paratenon pain are limited; there is indirect support from Achilles tendinopathy and general HILT data. Due to the superficial target, 980 nm and, in selected cases, 1470 nm may be considered as an adjunct.

Short answerThe Achilles paratenon is the superficial tissue surrounding the tendon. Direct, strong clinical data for laser in paratenon-derived pain and tenderness is limited. High-power laser studies in Achilles tendinopathy provide indirect support.

Symptoms

Superficial tenderness around the Achilles, a friction sensation, pain with exercise, and sometimes local swelling can be seen.

980 and 1470 nm

980 nm is the main adjunctive wavelength for the tendon and surrounding tissues. Because the paratenon is quite superficial, 1470 nm may be anatomically accessible; however, there is no direct efficacy data, and the thermal load must be carefully controlled.

Scientific evidence

Level of evidence: C+. There is a high-power laser RCT for Achilles tendinopathy; paratenon-specific evidence is limited.

Our approach in our Bodrum clinic

We differentiate paratenon pain from midportion tendinopathy, insertional pain, and partial tear; we add laser to load management and calf strengthening.
