Distinguishing ear diseases

Outer ear canal infection, middle ear infection, fungal infection, dermatitis, eardrum perforation, and cholesteatoma are distinct from one another. Before considering otic ozone, an otoscopic evaluation helps determine the safe target for local application.

Application principle

In otic ozone, the goal is to provide controlled local contact around the outer ear without ozone being delivered into the respiratory tract. The equipment and application technique used are selected to prevent ozone from escaping into the environment.

Human data

Human studies from 1998 and 2009 reported favorable clinical and microbiological outcomes when ozone gas or ozonated preparations were added to combination therapy. Although these studies were conducted prior to modern standards for large randomized trials, they provide direct human data investigating the potential of local ozone in ear infections.

New research

In 2025, a controlled veterinary study was published demonstrating strong results for topical medical ozone in otitis externa. These data cannot be directly translated to human treatment; however, they raise new research questions regarding local antimicrobial and tissue responses and support the advancement of human studies.

Evaluation alongside standard treatment

For acute otitis externa, accurate diagnosis, pain control, and appropriate topical treatment constitute the fundamental approach. If an appropriate target exists, local ozone may be considered as a complementary option supporting this treatment plan.

Eardrum safety

A history of perforation or middle ear surgery alters local treatments. For this reason, otic ozone should not be administered at home or without an otoscopic evaluation; local anatomy and the condition of the eardrum must be evaluated beforehand.
